Address Change

If you are an employee with an active job at SMU, you can update your Home and Mailing addresses online via the Employee Self Service area of under Personal Information. Be sure to update both addresses as appropriate.

If you are unable to update your information via Self Service, you may submit the changes on paper using one of the following forms:

Address Change Form (for temporary employees including adjunct faculty and temporary staff) - Submit the completed form to the Department of Human Resources.

Faculty Personal Data Form (for regular faculty) - Submit completed form to the Department of Human Resources.

Staff Personal Data Form (for regular staff) - Submit the completed form to the Department of Human Resources.

SMU students also should use the self service feature within  to submit address changes.
